Progressive Eye Care patients age 18 or older are eligible to participate. Previous winners, employees of Progressive Eye Care and members of their families are not eligible.
A referred patient MUST complete an eye exam before entry or prizes are given to the referring party.
Winners will be selected in a random drawing from all eligible entries received by the last day of the month prior to the announced drawing. Odds of winning depend on the number of eligible entries received. Winners will be notified by phone or email within 7 days of the drawing date. Each winner must comply with all terms conditions set forth in these rules, and winning is contingent upon fulfilling all requirements.
Progressive Eye Care makes no warranties with regards to prizes. No substitutions of prizes are allowed. Prizes cannot be exchanged for cash.
By participating, all winners will grant Progressive Eye Care exclusive permission to use their names, photographs, voices and likenesses in connection with this promotion and any future contests, advertising and marketing including Social Media for Progressive Eye Care.
By participating each winner waives any and all claims of liability against Progressive Eye Care, it's employees and agents for any personal injury or loss, harm, damage, cost or expense, including travel, for participating in contest or from any injury or harm from the prize itself.
Program is subject to change without notice. Tickets are discarded at the end of each year and the program starts over on January 1st of each year.
